急性冠脉综合征炎性标志物hsCRP与sVCAM-1的表达及临床意义

摘    要: 目的 观察不稳定性心绞痛(UAP)和急性心肌梗死(AMI)患者血清中超敏C反应蛋白(hs-CRP)和可溶性血管细胞黏附分子(sVCAM-1)水平变化,探讨炎症标志物与急性冠脉综合征(ACS)发病及病变程度的关系与临床意义.方法 采用酶联免疫吸附测定法检测50例ACS患者和30例正常对照组血清中hs-CRP和sVCAM-1水平,所有入选者均行冠脉造影检查,记录血管病变情况.结果 血清hs-CRP和sVCAM-1浓度在AMI组和UAP组均明显高于对照组(P<0.01),且AMI组明显高于UAP组(P<0.01);血清sVCAM-1与受累冠状动脉血管支数呈正相关,r=0.450,P<0.01.而hs-CRP水平与受累冠状动脉血管支数无相关关系,r=0.245,P>0.05.结论 外周血hs-CRP、 sVCAM-1与急性冠脉综合征发病密切相关,且sVCAM-1水平可反映冠状动脉病变程度.

Abstract:Objective To investigate the relation of inflammation markers to acute coronary syndrome(ACS)and their clinical significance by observing the changes of hs-CRP and VCAM-1 in patients with unstable agina pectoris(UAP) and acute myocardial infarction(AMI).Methods The concentrations of serum hs-CRP and sVCAM-1 were assayed by enzyme linked immunosorbant assay,the severity of coronary atherosclerosis was evaluated by coronary angiography and recorded in 50 patients with ACS and 30 controls.Results The concentrations of serum hs-CRP and sVCAM-1 were significantly higher in ACS patients than those in controls(P<0.01),and significantly higher in AMI patients than those in UAP patients(P<0.01).There was significant positive correlation between the concentration of serum sVCAM-1 and the lesion extent of involved coronary artery(r=0.450 P<0.01).Conlusions Serum hs-CRP and sVCAM-1 levels are closely correlated with the development of ACS,and the concentrations of serum sVCAM-1 reflect the lesion extent of coronary artery.
