二尖瓣狭窄闭式扩张分离320例临床分析

对320例风湿性心脏病二尖瓣狭窄闭式扩张分离术的疗效进行了分析,术后优良进步308例(96.3％)。发现影响手术疗效的主要因素为病变程度、心脏功能、瓣口返流和心房纤颤。强调了手术入路和闭式扩张分离的适度。指出了闭式扩张分离术仍是一种治疗风心病二尖瓣狭窄简便易行的有效方法。如能严格掌握手术指征,加强术前准备、术中处理和后续治疗,将会使疗效进一步提高。

CLOSED COMISSUROTOMY FOR MITRAL STENOSIS:CLINICAL ANALYSIS OF THREE HUNDRED AND TWENTY CASES

The result of closed comissurotomy for mitral stenosis was analysed in 320 cases of rheumatic heart disease. Of them, there were 308 (96.3%) patients with an excellent result after the operation. It was found that the main factors which influenced the result of surgical treatment were the extent of pathologic change, the cardiac function, the mitral regurgitation and the atrial fibrillation. The proper way of operation and the appropriate measure of closed comissurotomy are stressed. It is pointed out that the closed comissurotomy remains a simple, easy, effective method for rheumatic mitral stenosis. The result of the surgical treatment will further be improved if the measures are adopted of the strict grasp of operative indication and the enhancement of preoperative preparation, proper management during operation and continuous postoperative treatment.
