宫腔镜诊治不孕症的临床价值

目的探讨宫腔镜在诊断和治疗女性不孕症方面的临床价值。方法回顾性分析3126例不孕症患者宫腔镜检查病例。结果原发不孕1265例,发现宫腔病变536例（42.37%）,按发病率前3位分别是子宫内膜息肉221例（17.47%）、子宫内膜息肉样增生151例（11.94%）、不全纵隔子宫62例（4.90%）。继发不孕1861例,发现宫腔病变552例（29.66%）,按发病率前3位分别是子宫内膜息肉170例（9.13%）、子宫内膜息肉样增生134例（7.20%）、宫腔粘连89例（4.78%）。原发不孕症与继发不孕症的宫腔情况在子宫内膜息肉、子宫内膜息肉样增生、不全纵隔子宫、宫腔粘连、子宫发育不良、正常宫腔等方面比较差异有显著性（P〈0.001）。子宫内膜息肉组、子宫内膜息肉样增生组、不全纵隔子宫组、宫腔粘连组、正常宫腔组术后妊娠率分别是46.91%、24.83%、38.46%、65.22%、12.88%。结论不孕症患者中宫腔疾病发病率高,宫腔镜是检查及治疗宫腔因素引起不孕症的可靠方法。

The Clinical Value of Hysteroscopy in Ddiagnosis and Treatment of Infertility

Objective To evaluate the clinical value of hysteroscopy in the diagnosis and treatment of female infertility.Methods Retrospective analysis of 3126 cases hysteroscopy examination of infertility.Results 1265 cases of primary infertility,intrauterine lesions found in 536 cases（42.37%）,according to the incidence of endometrial polyps in the top three were 221 cases（17.47%）,endometrial polyps in 151 cases of hyperplasia（11.94%）,incomplete septate uterus,62 cases（4.90%）.1861 cases of secondary infertility and found 552 cases of intrauterine lesions（29.66%）,according to the incidence of the top three were 170 cases of endometrial polyps（9.13%）,endometrial polyp-like hyperplasia in 134 cases（7.20%） intrauterine adhesions in 89 cases（4.78%）.Primary infertility and secondary infertility of uterine situation in endometrial polyps,endometrial polypoid hyperplasia,incomplete septate uterus,intrauterine adhesions,uterine hypoplasia,normal Intrauterine and so there was significant difference between（P0.001）.Endometrial polyp,endometrial polyp-like hyperplasia,incomplete septate uterus group,intrauterine adhesions,normal intrauterine pregnancy after operation were 46.91%,24.83%,38.46%,65.22%,12.88%.Conclusion Infertility rate in patients with Uterine diseases is high,hysteroscopy is a reliable way in the examination and treatment of infertility caused by intrauterine factors.
