老年重型颅脑损伤肺部并发症的诊断与治疗

目的　总结老年颅脑损伤后肺部并发症的救治经验。方法　回顾我科 1995 - 0 1～ 2 0 0 0 - 12收治的 134例老年重型颅脑损伤临床资料。结果　本组肺部并发症 5 7例 ,治愈 36例 ,2 1例死于呼吸功能衰竭。结论　对老年重型颅脑损伤患者尽早地清除血肿、大骨瓣减压、系统地呼吸功能监护、早期气管切开和机械辅助呼吸是减少并发症和降低死亡率的重要手段。

Diagnosis and Treatment of Pulmonary Complication Following Senile Severe Head Trauma

Objective To summarize the theraputic experience of pulmonary complication following senile severe head trauma.Methods 134 senile patients with severe head trauma,who admitted to our department from Jan 1993 to Dec 2000 were analyzed retrospectively.Result Of the 57 cases diagnosed as pulmonary complication,36 were cured,21 finally died of respiratory failure.Conclusion Early evacuation of intracranial hematoma with large decompressive cranietomies,systematic respiratory monitoring,early trachectomy and mechanical ventilation are effictive measures for reduction of mortality and pulmonary complication following senile head trauma.
