脊柱外伤的MRI对临床诊治的作用

目的:了解脊柱外伤的MRI表现与其临床价值。材料与方法:对37例脊柱外伤的MRI征象进行回顾性分析,并与临床和手术结果进行比较。结果:脊柱外伤的MRI征象可分为:1)椎旁软组织损伤5例(13.5％);2)椎后韧带损伤7例(18.9％);3)椎体骨折37例(100％);4)椎间盘损伤7例(18.9％);5)椎管内异常(包括硬膜和脊髓受压,脊髓损伤)32例(86.5％)。临床症状的轻重和预后与脊髓损伤类型和引起脊髓信号异常的因素有密切关系,硬膜和/或脊髓受压,脊髓信号无异常者症状轻,预后好;压迫所致的脊髓水肿样信号,及时手术清除,预后良好;脊髓挫裂伤或伴小血肿形成、脊髓横贯性损伤及脊髓软化症状重,预后差。结论:MRI能反映脊髓损伤的各种病理改变,对临床的处理和预后判断具有重要价值。

Spinal Injury:MRI Findinds and Clinic

Purpose: To evaluate the MRI findings and clinical value of spinal injury. Materials and Methods: MRI features of spinal injuries in 37 cases were retrospectively analysed and were compared with the findings of their operations and clinical manifestations. Results: MRI findings were: 1) Paraspinal tissue injuries in 5 cases (13. 8%): 2) Ligamentous injuries in 7 cases (18.9%): 3) Verterbal fractures in all cases (100%): 4) Disc injuries in 7 cases (18.9%): 5) Intraspinal abnormalities in 32 cases (86.5%). MRI could show the causes of cord compression and signal abnormalities which were closely related with their manifestation and prognosis. Conclusion: MRI could find a variety of pathological changes of spinal injury and plays an important role in clinical treatment and in. predicting prognosis.
