血脂水平与脾功能关系的初步探讨

目的探讨生理状态下脾功能与血脂水平的关系。方法261例心内科住院患者血涂片镜检计数豪一周小体（Howell-Jollybodies），检测TC、TG、LDL-C、HDL-C。统计每周乙醇摄入量（WAI）、每周运动次数（WEF）、降脂药物应用情况（LDA），计算体重指数（BMI），根据患者膳食情况计算膳食评价分数（DES）。对血脂水平与豪一周小体计数（HJC）进行双因素及多因素分析。结果双变量分析显示TC、TG、LDL-C与HJC呈正相关，r=0．201（P〈0．01），r=0．132（P〈0．05），r=0．325（P〈0．01），HDL-C与HJC呈负相关r=-0．268（P〈0．01）。多元逐步回归分析，TC、TG、LDL-C与HJC呈独立正相关，HDL-C与HJC则无相关关系。结论血脂水平与脾功能密切相关，脾功能减退是脂质代谢紊乱的独立毹除因素。

Discussion on the association of serum lipid level with splenic function

Objective To investigate the association of serum lipid level with splenic function. Methods 261 patients underwent blood smear examination, and HowellJolly bodies counted, meanwhile TC, TG, LDL-C and HDL-C were tested. Weekly alcohol intake （WAI）, weekly exercise frequency （WEF）, lipidlowering drug application （LDA）, and compute body mass index （BMI） were determined. Dietary evaluation score （DES） was calculated according to patients dietary habits. The association of lipids level and HJC was analyzed by linear correlation and multiple linerstepwise regression. Results Linear correlation revealed that TC, TG and LDL-C all had positive correlation with HJC, r=0.201 （P〈0.01）, r=0.132（P〈0.05）, r=0.325（P〈0.01） respectively, however HDL-C had negative correlation with HJC, r=-0.268（P〈0.01）. Multiple linear stepwise regression analysis showed that TC, TG and LDLC had positive correlation with HJC, otherwise HDLC had no correlation with HJC. Conclusion There is an intimate relationship between lipid level and HJC, hyposplenism is probably an independent risk factor of lipid metabolism disorders.
