| 摘 要： | 我国台湾面对人口结构老龄化，在老年长期照护实践中开展了多专业团队服务形式。文章针对多专业团队的概念和运作方式及专业人员角色、职责、服务内容和形式进行了阐述，并探讨了多专业团队在实践中的应用及实施个案管理的经验与效果。 |

| Abstract: | Taiwan has launched the muhidisciplinary team model in the long - term care practice for the aged to face with the challenge of ageing popula-tion. This paper summarized the concept of muhidiseiplinary team and its operation model. Besides, the roles, duties and service contents of professional staffs were also introduced. Moreover, practice of muhidisciplinary team and experiences and effects of case management were discussed. |
