肾癌的诊断与治疗(附60例报告)

目的：探讨肾癌的诊断与治疗。方法：对1989～2000年收治的肾癌60例，结合文献对肾癌的发病、诊断和治疗方法作了分析，并讨论了影响肾癌预后的诸因素。结果：60例B超检查，正确率达93．3％；60例CT检查，诊断正确率100％；12例肾动脉造影，诊断正确率100％。60例术后病理诊断，透明细胞癌38例，颗粒细胞癌16例，混合细胞癌4例，梭形细胞癌1例，未分化癌l例。单纯肾切除6例，部分肾切除l例，根治性肾切除5l例，无法手术仅取活检2例。60例中获随访40例，1年存活率87．5％，3年存活率60％，5年存活率33．3％。结论：强调早期诊断和早期行肾癌根治术的意义。

DIAGNOSIS AND TREATMENT OF RENAL CELL CARCINOMA

Objective:To discuss the diagnosis and treatment of renal cell carcinoma.Methods:Retrospective study the diagnosis and treatment of 60 patients with renal cell carcinoma in our hospital from 1989 to 2000,and discuss the prognosis factors. Results:CT and angiography detected positive findings in all patients while ultrasound did in 93.3%. The histologic types of renal cell carcinoma include: clear cell (38 cases); granular cell (16 cases); mixed type( 4 cases); sarcomatoid carcinoma( 1 case) ,and poor differentiaed type ( 1 case).Except 2 cases only had biopsy ,others undertook surgery: simply nephrectomy ( 6 cases );radical nephrectomy( 51 cases); partial nephrectomy( 1 case). Total 40 cases followed up. The 1-,3-,and 5-year survival rate are 87.5%,60%,and 33.3% respectively.Conclusions:Early diagnosis and radical nephrectomy for renal 'cell carcinoma should be emphasised.
