噬菌体生物扩增法检测结核分枝杆菌在诊断肺结核中的价值

摘    要:目的:研究应用噬菌体生物扩增法检测结核分枝杆菌在肺结核诊断中的价值。方法:应用噬菌体生物扩增法对416例肺结核和168例非结核性肺部疾病患者的痰标本进行检测,评价其对肺结核的诊断价值及同细菌学检查方法是否存在差异。结果:噬菌体生物扩增法诊断菌阳肺结核的敏感度为98.5%,诊断菌阴肺结核的敏感度为63.3%,诊断肺结核的总敏感度为75.0%;诊断肺结核的特异度为95.2%;噬菌体生物扩增法阳性检出率为75.0%,高于细菌学检查方法33.2%(P<0.005)。结论:噬菌体生物扩增法检测结核分枝杆菌在诊断肺结核中有较好的灵敏度和特异度,是辅助诊断和鉴别诊断肺结核的有效手段。

Abstract:Objective: To evaluate the Phage amplified biologically assay in the diagnosis of tuberculosis. Methods: Samples of sputum from 416 cases of tuberculosis, 168 patients with no-tuberculosis lung disease were detect using Phage amplified biologically assay to evaluate the significance in diagnosis of tuberculosis and compare the effectiveness of PhaB and method of sputum smear. Results: The sensitivity of PhaB in diagnosis of bacteriologically positive and negative pulomonry tuberculosis was 98. 5% and 63. 3%, repectivelly,the combined sensitivity was 75. 0%,and the specificity was 95.2%. The case detection rate of PhaB(75.0%) was significantly higher than that in method of sputum smear which was 33.2%(P〈0. 005). Conclusion: The results suggest that the measurement of Phage amplified biologically assay is a relatively sensitive and specific method for the diagnosis and differential diagnosis of tuberculosis.
