翼点入路显微手术切除大型垂体腺瘤

目的 报道经翼点入路显微手术切除45例大型垂体腺瘤。方法 经翼点入路开颅,采用显微手术解剖鞍区各脑池,充分利用鞍区4个间隙切除肿瘤。结果 45例中达全切除或次全切除40例,占89％,无手术死亡,无严重并发症,复发2例,占4.4％。结论 采用翼点入路显微手术切除大型垂体腺瘤,疗效满意。

Microsurgical Resection of Large Pituitary Adenoma via Pterional Approach

Objective To introduce the experiences in removal of large pituitary adenoma. Method Data of 45 cases with large pituitary adenoma, which were microsurgically operated via pterional approach, were reviewed retrospectively. Dissection of cisterns of the sellar region was carried out first under the operative microscope and the microsurgical ressections were then done by making use of the fully exposed anatomic spaces. Results In this series of patients, total removal of the tumor was achieved in 40, with no death of the operation or severe complication. Two patients relapsed during follow-up period. Conclusion The authors considered that satisfactory outcome could be expected when the pterional approach was used for microsurgical operation on patient with large pituitary adenoma.
