隧道式胸腔导管置入并胸腔内注药治疗癌性胸腔积液的临床对照研究

目的评价隧道式胸腔导管置入引流结合胸腔内灌注顺铂、高聚生治疗癌性胸腔积液的疗效。方法将53例单侧中至大量癌性胸腔积液患者随机分成两组,观察组27例采用隧道式胸腔导管置入法引流,对照组26例采用常规胸腔穿刺抽液,并均用硬化剂顺铂、高聚生注入胸腔。结果胸腔积液控制有效率分别为观察组85.19%（CR 8例,PR 15例）,对照组38.46%（CR 3例,PR 7例）,观察组优于对照组。观察组生活质量好转率优于常规穿刺组。结论采用隧道式胸腔导管置入引流结合胸腔注药治疗癌性胸腔积液有较好的疗效,且安全、不良反应少。

Tunnelled pleural catheter insertions and intrapleural administration of drugs for malignant pleural effusion: a clinical control study

Objective To assess the effect and safety of the treatment using a tunneled pleural catheter(TPC) and the intrapleural administration of cisplatin and highly agglutinative staphylococcin for malignant pleural effusion(MPE).Methods Fifty-three patients with moderate or large amount of ipsilateral malignant pleural effusion(MPE) were randomly divided into tunneled pleural catheter group(test group) and thoracentesis group(control group).Cisplatin and highly agglutinative staphylococcin(HASL) were infused into the thoracic cavity of the patients after drainage.Responses were evaluated 30 days later.Results Overall response rate(complete response plus partial response) was 85.19% in test group and 38.46% in control group(P<0.01).Improvement rate for the quality of life was 74.07% in test group and 30.77% in control group(P<0.01).The side effects in test group were less than that in control group.Conclusion Placement of tunneled pleural catheter for drainage and intrapleural administration of cisplatin and highly agglutinative staphylococcin is an efficacious therapy for patients with malignant pleural effusion.
