70岁以上COPD合并肺间质纤维化患者的临床观察

目的 了解70岁以上COPD并肺间质纤维化患者的临床特点,探讨其临床意义.方法 收集70岁以上COPD并肺间质纤维化患者共42例,掌握其肺功能、动脉血气分析及胸部高分辩CT（HRCT）的结果,同时随机收集了58例70岁以上COPD患者上述相关指标进行对照分析.结果 70岁以上COPD并肺间质纤维化患者与70岁以上COPD患者相比,病程长,肺功能进行性下降,低氧血症明显.结论 COPD并肺间质纤维化患者具有两种疾病的特点,临床症状重,体征明显,预后差.

Clinical observation of patients with chronic obstructive pulmonary disease complicated with pulmonary interstitial fibrosis older than 70 years

Objective To explore the clinical characteristics of elderly patients with chronic obstructive pulmonary disease （COPD） complicated with pulmonary interstitial fibrosis older than 70 years. Methods 42 elderly COPD patients complicated with pul- monary interstitial fibrosis and 58 elderly patients with COPD only were selected, who were all older than 70 years. Their clinical symp- toms, pulmonary function, arterial blood gas and high resolution computed tomography （HRCT） of chest were analyzed respectively. Re- suits Compared with the patients with COPD only, the patients with COPD complicated with pulmonary interstitial fibrosis had a longer disease history, lower pulmonary function and more serious hypoxemia. Conclusion The patients of COPD with pulmonary interstitial fi- brosis have the both characteristics of COPD and pulmonary interstitial fibrosis, they have a longer disease history, lower pulmonary func- tion and worse clinical symptoms.
