子痫前期与新生儿死亡相互关系的回顾性队列研究

目的：分析子痫前期与新生儿死亡之间的相互关系。方法：从1995～2006年间选入孕产妇9138例，其中子痫前期患者共1005例。按回顾性队列研究原则，采用多变量Logistic回归模型调整混杂因素后评估子痫前期对新生儿死亡率的影响。统计新生儿的早期（0～7d）及晚期（8—42d）死亡率。结果：子痫前期组的早产儿死亡率均较低（OR：0．60，95％CI：0．53—0．63及OR：0．82，95％CI．0．71-0．89），而足月新生儿的死亡率较高（OR：1．04，95％CI：1．01—1．13）。结论：子痫前期是早产儿死亡的低风险因素，但对足月新生儿具有高风险。

Relationship between pre-eclampsia and neonatal mortality:a retrospective cohort study

Objective:To assess the association between pre-eclampsia and neonatal mortality.Methods: A total of 9 138 puerperas between 1995 and 2006,including 1 005 lying-in women with pre-eclampsia,were submitted to this study.Both retrospective cohort study and multivariate logistic regression were applied to evaluate the association between pre-eclampsia and neonatal mortality,with adjustment of potential confounders.The early-stage(0-7 days) and late-stage(8-42 days) neonatal mortality were analyzed statistically.Results: There was a significant reduction in neonatal mortality associated with pre-eclampsia in preterm infants(OR: 0.60,95% CI:0.53-0.63 and OR: 0.82,95% CI:0.71-0.89),but a significant increase in term infants(OR: 1.04,95% CI: 1.01-1.13).Conclusion: Pre-eclampsia is associated with lower risk of infant death in preterm births but higher risk in term births.
