子痫前期与新生儿死亡相互关系的回顾性队列研究

摘    要:目的:分析子痫前期与新生儿死亡之间的相互关系。方法:从1995~2006年间选入孕产妇9138例,其中子痫前期患者共1005例。按回顾性队列研究原则,采用多变量Logistic回归模型调整混杂因素后评估子痫前期对新生儿死亡率的影响。统计新生儿的早期(0~7d)及晚期(8—42d)死亡率。结果:子痫前期组的早产儿死亡率均较低(OR:0.60,95%CI:0.53—0.63及OR:0.82,95%CI.0.71-0.89),而足月新生儿的死亡率较高(OR:1.04,95%CI:1.01—1.13)。结论:子痫前期是早产儿死亡的低风险因素,但对足月新生儿具有高风险。

Abstract:Objective:To assess the association between pre-eclampsia and neonatal mortality.Methods: A total of 9 138 puerperas between 1995 and 2006,including 1 005 lying-in women with pre-eclampsia,were submitted to this study.Both retrospective cohort study and multivariate logistic regression were applied to evaluate the association between pre-eclampsia and neonatal mortality,with adjustment of potential confounders.The early-stage(0-7 days) and late-stage(8-42 days) neonatal mortality were analyzed statistically.Results: There was a significant reduction in neonatal mortality associated with pre-eclampsia in preterm infants(OR: 0.60,95% CI:0.53-0.63 and OR: 0.82,95% CI:0.71-0.89),but a significant increase in term infants(OR: 1.04,95% CI: 1.01-1.13).Conclusion: Pre-eclampsia is associated with lower risk of infant death in preterm births but higher risk in term births.
