白细胞酯酶试验在诊断男性尿道炎中的作用

目的　评估白细胞酯酶试验 (LET)诊断男性尿道炎的价值。方法　对 12 9名男性受试者分别进行尿道试子NG培养、采集晨尿或长时间不排尿后的首段尿 (FVU)进行LET和CT的检测。结果　发现CT阳性 7例 (5 .43 %) ,NG阳性 10例 (7.75 %)。病征法诊断男性NG/CT感染的敏感性为 3 5 .2 9%,特异性为 97.3 2 %;LET诊断男性NG/CT感染的敏感性为 5 8.82 %,特异性为 95 .5 4%。结论　在本地区 ,对于有尿道分泌物的男性 ,可以参照WHO推荐的性病病征处理法进行相应的诊治。LET具有较高的诊断特异性 ,有助于发现无症状男性NG/CT感染者。

The Value of the Urine Leukocyte Esterase Test in Diagnosing the Urethritis in Male

Objective To evaluate the use of urine leukocyte esterase test(LET) in diagnosing the infection caused by Neisseria Gonorrhoeae(NG) and Chlamydia trachomatis(CT) in male.Methods Urethral swabs for NG culture as well as First void urine(FVU) for ligase chain reaction(LCR) and LET were collected from 129 males in a STD clinic in Beijing.Results 10 subjects were positive with NG.7 cases were positive with CT.The syndromic approach had a sensitivity of 35.29% and a specificity of 97.32% for the presence of NG or CT as well as the LET had a sensitivity of 58.82% and a specificity of 95.54% in men.Conclusion Syndromic Approach is recommended to use to the men who have urine discharge in this area.The LET has a high specificity and is a useful tool to find out asymptomatic men with infection of NG/CT.
