无钛夹迷你型腹腔镜阑尾切除临床研究

目的 探讨无钛夹及微型腹腔镜阑尾切除术的方法和应用价值。方法 1999年1～8月,随机抽取47例急慢性阑尾炎病人,无条件采用无钛夹迷你型腹腔镜阑尾切除术,取3mm、5mm、10mm三个穿刺孔,用10mm腹腔镜观察,沿阑尾缘电凝、电切阑尾系膜至根部,“0”号肠线双重套扎,切除阑尾,电凝阑尾残端粘膜,自10mm孔取出阑尾。结果 全部完成腹腔镜阑尾切除术,无中转开腹,平均手术时间57.1分钟,其中,完成无钛夹迷你型腹腔镜阑尾切除术43例,占91.5％,术中转为施钛夹4例,均顺利出院,平均住院日4.2天。结论 无钛夹迷你型腹腔镜阑尾切除术,适合于发病72小时内各种类型急慢性阑尾炎,创伤更小,恢复快,几乎不留疤痕,符合美学。更重要的是避免了因钛夹留置体内对病人心理及磁共振等检查的影响,该方法既安全可靠,又能降低费用,值得推广。

Results All the cases were completed successful. No patient was converted to open operation. Conclusion non - clip mini - laparoscopic appendectomy has many advantages: the injury is very little. the patients recovery is quickly. the expenditule is low, moreover, this method canreduce the patient' s psychological burden without disturbing magenetic resonance examination lately.
