脂肪肝程度进展对体检人群代谢指标的影响

摘    要:目的揭示脂肪肝进展过程中伴随的机体代谢变化及其对人体的影响,引起人们对脂肪肝的重视并进行有效的干预。方法选择2年内连续2次在我处进行健康体检,经B超确诊脂肪肝程度进展的体检者414名作为本次研究的对象。应用配对t检验及卡方检验的方法,对研究对象的体质指数(BMI)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、空腹血糖(FPG)、血尿酸(UA)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL—C)等体检指标的水平及异常率进行比较。结果除FPG外,脂肪肝进展前后的BMI、TG、HDL—C、ALJT、AST、UA的水平及异常率均有统计学差异,P〈0.05;脂肪肝进展后BMI、TG、ALT、AST、UA的水平较前增高,HDL—C的水平较前降低,上述指标的异常率均呈现增高趋势。结论脂肪肝并非单纯的肝脏病变,脂肪肝的进展伴随着一系列代谢指标的改变,因而对脂肪肝的防治除了要重视对单纯性脂肪肝的早期干预,更要注重对代谢综合征的防治。

Abstract:Objective To investigate the relationship between fatty liver progress and metabolic changes. Methods A total of 414 patients who had B-mode uhrasonography confirmed fatty liver development during 2 health check-ups within 2 years were enrolled in this study. Paired t and χ^2 tests were used to compare body mass index (BMI), alanine aminotransferase (ALT), aspartate aminotransferase (AST) , fasting plasma glucose ( FPG ) , serum uric acid ( UA ) , triglycerides ( TG ) , and high-density lipoprotein cholesterol ( HDL-C ) among the participants. Results BMI, TG, HDL-C, ALT, AST, and ILIA were significantly increased after fatty liver progressed, although HDL-C was largely decreased. Conclusions Prevention and treatment of MS as well as early intervention for fatty liver should be important for successful control of fatty liver.
