肝内胆管细胞癌临床特点分析

目的肝内胆管细胞癌是原发性肝癌的一种较少见亚型,通过对83例临床特点的分析以加深对此种疾病的认识。方法回顾性分析1998年1月~2001年1月经手术治疗的83例肝内胆管细胞癌病人的临床资料,并根据合并肝硬化与否分为两组,对临床资料进行比较。结果83例中31例合并肝硬化患者的平均年龄46岁,HBsAg阳性率87%(27/31),合并门静脉癌栓32%(10/31),淋巴结转移36%(11/31),CA19-9阳性81%(25/31),与不合并肝硬化患者相比差异显著(P<0.05),两组1、3、5年生存率分别为31%、11%、2%和67%、45%、16%,差异有统计学意义(log-rank值14.85,P<0.001)。结论合并有肝硬化的肝内胆管细胞癌具有发病年龄轻,HBsAg阳性率、CA199阳性率、门脉癌栓发生率、淋巴结转移率高以及预后差的特点。

CLINICAL PROFILE OF CHOLANGIOCARCINOMA

Objective To investigate the clinical profile of cholangiocarcinoma(CC).Methods 83 patients with CC(52 with cirrho and 31 without cirrhosis underwent operation in our department from Jan.1998 to Jan.2001,and their data were analyzed retrospectively.Results The mean age was 46 years in cirrhosis group.The 65 years in non-cirrhosis group,and positive rate of HBsAg,CA199,portal vein thrombosis and lymph node metastases in cirrhosis group were 87%,81%,32% and 36% respectively,which were higher than that of non-cirrhosis group.The 1-,3-,and 5-year survival rate of cirrhosis group and non-cirrhosis group were 31%,11%,2% and 67%,45%,16%(log-rank=14.85,P<0.001)respectively.Conclusions CC in cirrhotic liver presented a younger age and high rate of positive serum HBsAg,positive CA199,portal vein thrombosis and lymph node metastases.Their survival period was shorter than that of patients without cirrhosis.
