顺铂加5-Fu诱导化疗后放疗与单纯同期放疗治疗中晚期鼻咽癌效果比较

目的探讨顺铂加5-FU诱导化疗后放疗与单纯同期放疗治疗中晚期鼻咽癌效果比较。方法依据治疗方法不同分为联合组42例、放疗组42例观察两组疗效。结果两组病例近期疗效观察,联合组鼻咽部病灶及颈部转移淋巴结病灶治疗总有效率100%,显著优于放疗组95.24%、92.85%,经统计学分析,有显著差异性。结论 95%鼻咽癌病理为低分化鳞癌和未分化癌,具有生长快、恶性程度高,易发生血道、淋巴结转移,明确诊短时有75%患者已经发展至中晚期鼻咽癌;由于鼻咽癌特殊的解剖位置,肿瘤位于头中央,大量血管和神经在肿瘤周围分布,手术不能够彻底治疗,并且鼻咽癌对放射线敏感,放射治疗是鼻咽癌首选治疗方法。应用放射治疗前诱导化疗可以显著的降低肿瘤负荷,缩小肿瘤体积,提高放疗效果,减少肿瘤发生远处转移,从而延长生存期。

Radiotherapy after induction chemotherapy with cisplatin plus 5-FU and simple concurrent radiotherapy for advanced nasopharyngeal carcinoma

Objective To investigate the effect of radiotherapy alone over the same period and radiotherapy after induction chemotherapy with cisplatin plus 5-Fu in advanced nasopharyngeal compare.Methods According to the different treatment was divided into 42 cases of the combined group,radiotherapy group,42 cases were observed efficacy.Results Term efficacy of the two groups of patients,the the nasopharyngeal lesion and neck combined group metastatic lymph node lesion treatment efficiency of 100%,significantly better than the radiotherapy group,95.24%,92.85%,and the statistical analysis,there are significant differences.Conclusion 95% of nasopharyngeal pathology poorly differentiated squamous cell carcinoma,and undifferentiated carcinoma with faster growth,high degree of malignancy,prone to the blood stream,lymph node metastasis,Ming diagnosed short time,75% of the patients have been developed to the advanced nasopharyngeal carcinoma,nasopharyngeal special anatomical location,tumor located in the head of the central,a large number of blood vessels and nerves around the tumor distribution,the surgery can not be definitive treatment,and nasopharyngeal carcinoma sensitive to radiation,radiation therapy is the preferred method of treatment of nasopharyngeal carcinoma.Induction chemotherapy before radiation therapy can significantly reduce tumor burden,reduce the tumor volume,the effect of radiation therapy to reduce the tumor distant metastasis,and thus prolong survival.
