腮腺常见良性肿瘤的CT诊断

目的分析腮腺常见良性肿瘤C4影像特征,探讨其表现。方法回顾分析2007年4月至2012年6月19例经手术病理证实的腮腺常见良性肿瘤的CT影像资料,对腮腺常见肿瘤重点观察部位、密度,进行定位,定性分析,比较与其影像相关良性非肿瘤性病变C4鉴别要点。结果其中16例为原发性腮腺混合瘤,1例脂肪瘤,1例淋巴血管瘤,1例鳃裂囊肿。14例形态为卵圆形或圆形,直径3～6.8cm,5例呈分叶多结节形,大部肿瘤边缘清晰光整;2例内部密度不均,钙化2例,脂肪密度1例。结论边缘清晰光滑,密度均匀或不均匀,增强扫描实性部分有不同程度均匀或不均匀强化,生长缓慢,短期内可迅速增大,手术复发率高,有助于诊断腮腺混合瘤;显示强化的血管团或血窦及呈簇状分布斑状钙化（静脉石）为腮腺淋巴血管瘤的特征性CT影像学特征;肿块为极低密度的脂肪组织充填是腮腺脂肪瘤的特征CT表现,诊断不难,无需鉴别;鳃裂囊肿属先天性病变,CT值0～20hu,与单纯性囊肿相似,结合临床有助诊断。

CT diagnosis of parotid common benign tumo

Objective To analysis parotid common benign tumor CT imaging characteristics.Methods discusses the performance.Material and methods retrospective analysis in April 2007~2012 June 19 cases of pathologically proven by surgery parotid common benign tumor of CT image data for tumor CT positioning,qualitative analysis,key observation,tumor location,density,analyses and compares the CT imaging characteristics,its image correlation benign lesions CT identify the neoplastic points.Results In 16 of these patients as primary parotid mixed tumors,and 1 case lipoma,1 case lymphatic hemangioma,1 case branchial cleft cysts;.14 cases for ovoid or round shape,diameter 3~6.8 cm,5 cases of points leaf multinodular shape,most tumor edge clear light whole.2 cases internal density is uneven and calcification in 2 cases,fat density 1 case.Conclusion Edge clear smooth,density or uneven,enhance the scanning solid parts have different level uniform or uneven strengthening,slow growth,short-term inside can be increased rapidly,surgery,is helpful to the diagnosis of high recurrence parotid mixed tumors,Display strengthened the arteriolar or blood sinus and show clumps distribution erythema shape calcification（vein for parotid lymph hemangioma stone） characteristic CT imaging characteristic.Bump into extremely low density of adipose tissue filling is characteristic of parotid gland lipoma,CT manifestations,no need to identify diagnostic easy,congenital lesions branchial cleft cysts;genera,CT value 0～20 hu,with simple cysts similar,combined with clinical help diagnosis.
