摘    要:目的 观察部分液体通气治疗肺灌洗诱导的急性肺损伤家猪时,肺气体交换参数的变化。方法 16只健康家猪,随机分为部分液体通气组(PLV)及机械通气组。机械通气组于急性肺损伤(ALI)后进行呼吸机常规机械通气;PLV组动物于ALI后气管内灌入氟碳化合物(perfluorocarbon,PFC,FC77,3M)30mL/kg。二组动物每小时均作动脉血气分析,观察氧分压(PaO2)、动脉血氧饱和度(SaO2、肺泡动脉氧分压差(AaDO2)、肺内右向左分流(Qs/Qt)和pH值,4h后处死动物。结果 部分液体通气组PaO2及SaO2显著改善,AaDO2及Qs/Qt显著降低。结论 以氟碳化合物为呼吸媒介的部分液体通气可明显改善肺灌洗诱导的急性肺损伤家猪的肺气体交换。

Abstract:Objective] To observe the effect of partial liquid ventilation (PLV) with perfluorocarbon (PFC) on lung gas exchange in the piglets with surfactant depletion-induced acute lung injury (ALI). Methods] 16 pigs were treated by lung lavage to partial pressure of oxygen in arterial blood (PaO2) below 100 mmHg for one hour and randomly divided into mechanical ventilation group (MV group) and partial liquid ventilation group (PLV group). Rabbits in PLV group received PFC (FC-77) intratracheally at doses of 30 mL/kg. The parameters of gas exchange were measured before lung lavage and after. When the ALI was established, and 1 hour, 2 hours, 3 hours, 4 hours after ALI. Animals underwent euthanasia at the end of the study. Results] Surfactant depletion induced by lung lavage formed a stable acute lung injury. PaO2 and SaO2 in PLV group was increased significantly with AaDO2 and Qs/Qt decreased synchronously. Conclusion] Partial liquid ventilation with perfluorocarbon improved the gas exchange in piglets with surfactant depletion-induced acute lung injury,
