电视胸腔镜腹腔镜联合下食管癌手术的体会

目的:探讨电视胸腔镜腹腔镜联合下食管癌手术的可行性。方法:术前准备同常规开胸手术。分3步操作,第1步:胸腔内食管游离和淋巴结清扫;第2步:腹腔镜下胃游离及淋巴结清扫;第3步:颈部食管游离及食管胃吻合。结果:本组3例无中转开胸者。手术时间5~7h,术中出血200~400m l,术后均顺利恢复,无明显并发症发生。结论:此手术安全可行,可逐步推广。

Laparoscope and video-assisted thoracoscopic surgery in esophageal carcinoma

Objective:To study the feasibility of using laparoseope and video-assisted thoracoscopic surgery(VATS)in radical correction of esophageal carcinoma.Methods:The preparation same as conventional tnoraeotomy.Operation included three stages.First stage:the dissociation of chest section of esophagus and lymph node scavenging under thoracoscope.Second stage:gastral dissociation and lymph node scavenging under laparoscope.Third stage:the dissociation of fcervical section of esophagus and inosculation of esophagus and stomachus.Results: Three cases was operated under VATS and laparoscope.There was no thoracotomy case.Operating time was in 5-7 hours,average amount of bleeding was in 200-400ml,all cases postoperative recovery were successfull.There was no complicatoin in all cases.Conclusions: This operation is safe and should be applied generally step by step.
