磁共振尿路成像在尿路梗阻性疾病中的应用

目的：评价磁共振尿路成像(MRU)在尿路梗阻性疾病中的诊断价值。方法：对25例尿路梗阻的患者，进行MRU检查，结合B超、静脉尿路造影(IVU)及手术和病理诊断，进行回顾性分析。结果：输尿管肾盂连接部狭窄7例，肾和输尿管重复畸形5例，肾盏憩室1例，梗阻型巨输尿管1例，输尿管手术后输尿管扭曲成角或狭窄3例，MRU均诊断明确；输尿管结石6例，MRU可明确梗阻部位，其中4例显示出无信号的结石影；输尿管癌1例，肾盂癌1例，MRU未能做出诊断。结论：MRU对尿路梗阻性疾病具有很高的定位诊断价值，尿路形态显示满意；对梗阻原因的定性诊断，存在一定的局限性。

MR Urography in the Diagnosis of Urinary Tract Obstruction

Objective:To evaluate the MR urography in the diagnosis of urinary tract obstruction.Methods:Twenty-four patients with urinary tract obstruction shown by ultrasonic examinaton first were examined by MRU.We combined the data of operation,pathology and other images,and made a retrospective analysis.Results:MRU gave a positive diagnosis of all the cases.They were associated with PUJO (5),congential malformation of the kidney and ureter(6),calyx diverticulum(1),ureterostensis after ureterotomy (4),calculus of ureter(6),renal pelvis carcinoma (1) and ureter carcinoma (1).The images of the ureter tract could be clearly shown.MRU was successful in localizing the lever of obstruction,but it could not reliably determine the cause of obstruction in renal pelvis and ureter carcinoma,and in 2 of 6 cases with calculus of ureter.Conclusion:MRU has a very high value in localizing the urinary tract obstruction,but it has its limitations on the understanding of the pathology some cases.
