内置式牵引成骨技术治疗颌骨畸形

摘    要:目的 :探讨内置式牵引成骨技术在不同类型的牙颌面畸形治疗中的应用。方法 :应用 2种类型的内置式牵引装置 ,为严重的小颌畸形、上颌后缩畸形、半侧颜面萎缩畸形和下颌骨缺损畸形患者行下颌骨体水平方向延长、上颌骨水平前徙、下颌升支与上颌骨同期垂直向延长以及下颌骨体部双端式牵引延长 ,共 5例。结果 :5例牵引成骨手术 ,部分病例辅助正颌外科手术 ,均达到术前设计要求 ,临床效果满意。 2例因牵引方向导致的错牙合问题 ,在牵引结束时及时得以纠正。无伤口裂开、感染、骨坏死、骨不连等并发症。结论 :应用内置式牵引成骨技术可重建面部轮廓 ,恢复理想的咬合关系和口颌功能 ,创伤小 ,安全可靠

Abstract:Objective:To describe the application of intraoral di st raction osteogenesis for correction of maxillofacial and mandibular deformities . Methods: 5 cases with serious maxillofacial or mandibula r deformities underwent distraction by using 2 types of intraoral distraction d evice.Mandibular lengthening and maxillary widening for micrognathia and maxilla ty constriction was performed in 1 case with Treacher Collions syndromes. M axillary distraction osteogenesis for maxillary hypoplasia in 2 cases.Simultaneo us maxillary and mandibular distraction in 1 case with hemifacial microsomia. T ransport distraction for mandibular body rectangle defect in 1 case. Results: Satisfactory morphologic results were achieved in all patients with good facial symmetry, ad equate occlusal relationship without any complication.Conclusions: Intraoral distraction osteogenesis provides a safe and effective treatment for the reconstruction of facial morphologic and occlusal relationshi p.
