十二指肠损伤术式选择

目的探讨十二指肠损伤的手术方法，以提高十二指肠损伤的外科治疗水平。方法对1993年10月～2003年11月我科收治的30例十二指肠损伤病例的临床资料进行回顾分析。结果单纯十二指肠损伤占20％(6／30)，合并其它腹腔内脏器损伤占86．67％(26／30)。损伤部位以降部多见．占46．67％(14/30)，水平部次之，占30％(9／30)。术后并发症发生率60％(18／30)．死亡率20％(6／30)。十二指肠瘘、胰瘘为主要并发症。结论根据术中损伤情况选择简单而合理的手术方式是提高治愈率、降低病死率的关键。

Discussion of Surgical Operation for Duodenal Trauma

Objective To investigate surgical operation for duodenal trauma to improve surgical treatment in patients suffering from duodenal trauma. Methods Clinical data of 30 cases of duodenal trauma from October 1993 to November 2003 were collected and retrospectively analysed. Results The pure duodenum injury 20%(6/30),86.67% (26/30) cases were complicated with additional other intraabdominal organ injures .The trauma was mostly often seen on the decending part 46.67%(14/30),and on the horizontal part 30%(9/30). The postoperation complication rate occurred in 60%(18/30),the mortality rate 20%(6/30). Duodenal fistula and pancreas fistula was the main complication.Conclusion The simple and reasonable surgical procedure based on the condition of duodenal injuries contribute to the better prognosis and low mortality rate of the patients with duodenal trauma.
