先天性支气管闭锁的CT与MRI诊断

摘    要:目的分析先天性支气管闭锁的CT和MRI表现,提高对该病的认识。方法对6例先天性支气管闭锁患者行CT和MRI检查。东芝64层CT和西门子3TMRI扫描仪扫描后原始数据均传至工作站行后处理,均行支气管树成像及多平面重建,主要观察病变的部位、形态、密度(或信号)和周围肺组织的改变。结果6例病例CT均显示黏液栓及肺气肿改变,MRI不能显示肺气肿改变,但对黏液栓显示更佳,均表现为稍短T1长T2信号。4例发生于左侧,2例发生于右侧。结论支气管黏液栓和局限性肺气肿征象是先天性支气管闭锁的典型影像学表现,黏液栓MRI信号具有特征性,CT扫描后处理MPR和支气管树成像技术、MRI冠状位及矢状位扫描对该病的诊断能提供更多信息。

The Diagnosis of Congenital Bronchial Atresia by CT and MRI

Abstract:Objective To present the CT and MRI findings of congenital bronchial atresia (CBA)in order to improve the diagnosis of CBA. Methods Six patients with CBA were performed on Toshiba 64 CT and Siemens 3T MRI,and postprocessing techniques were made at each dedicated workstations. The involved location of bronchial mucus plug, shape of mucus plug and emphysema around mucus plug were recorded. Results On CT findings,all 6 patients demonstrated bronchial mucus plug and peripheral emphysematous changes. The former was displayed better with short T1 and long T2 signal and the latter wasn't seen on MRI findings. Four CBA were in the left and two in the right. Conclusion The presence of bronchial mucus plug and emphysematous change is the typical radiologic finding of CBA. Postprocessing techniques such as MPR (muhiplanar reconstruction) and imaging of bronchial tree on CT, coronal section and sagittal section scan of MRI can provide more information for the diagnosis of CBA.
