CT对胃间质瘤恶性潜能高低判断的价值

目的分析胃间质瘤(gastric stromal tumor,GST)的CT表现,探讨其对GST恶性潜能的诊断价值。方法回顾性分析武汉大学人民医院2010年1月-2013年1月经手术病理确诊的38例GST患者的CT表现,并将其与术后病理学恶性潜能对照分析。结果病理学结果显示低度恶性潜能17例,高度恶性潜能21例。CT结果显示低度恶性潜能GST通常直径≤5 cm,肿块密度均匀,与周围组织界限清楚;高度恶性潜能GST多为直径5 cm、肿瘤密度不均匀,伴有坏死或液化,与周围组织界限模糊。出现转移的2例均为高度恶性潜能。结论 CT显示肿瘤大小、密度、与周围组织关系及是否转移等对于判断GST恶性潜能具有鉴别意义。

Value of CT in differential diagnosis between low and highly malignant potential gastric stromal tumor

Objective To analyze the manifestation of gastric stromal tumor （GST） in CT examination and to investigate the clinical value of CT in the diagnosis of low and highly malignant potential.Methods CT data of 38 patients confirmed by surgeiy and pathology in Renmin Hospital of Wuhan University from Jan.2010 to Jan.2013 were retrospectively analyzed,and the CT signs were compared with pathologic findings.Results In 38 cases of GST,17 cases were low malignant potential,and 21 cases were highly malignant potential.The low malignant potential lesions were usually below 5 cm in size,well-defined and homogeneous density.The highly malignant potential lesions were ill-defined,over 5 cm in size and inhomogeneous in density.Two cases of highly malignant potential tumor transferred to other tissue.Conclusion CT imaging shows the tumor diameter,homogeneous or inhomogeneous density,clarity of tumor margin,and transfer to other tissue are worthiness in differential diagnosis between low and highly malignant potential GST.
