腹腔镜肝切除35例临床分析

目的:探讨腹腔镜肝切除(LH)的安全性、可行性和微创性.方法:将2002年11月～2006年10月间行腹腔镜肝脏切除术的35例,与同时期开腹肝切除(OH)的病例42例对照研究,比较手术时间、术中出血量、术后进食时间、术后住院天数、术后并发症、手术前后肝功能的变化、出院时疾病转归和术后3d平均腹腔引流量等.结果:LH组手术时间长于OH组,术中出血量、术后并发症、出院时转归无显著差异;LH术后进食时间及住院天数比OH组短,LH组白蛋白、丙氯酸转氨酶改变比OH组小.结论:腹腔镜肝切除有创伤小、恢复快的优点,只要选择合适的病例,LH是安全、可行的.

A Clinical Analysis of 35 Cases of Laparoscopic Hepatectomy

Objective: To explore the safety and feasibility of laparoscopic hepatectomy (LH). Methods: 35 cases of LH and 42 cases of open hepatectomy (OH) were observed from Nov, 2002 to Oct, 2006. The indexes included operating time, operative bleeding volume, time of postoperative food intake, days of postoperative hospitalization, postoperative complications, liver function changes before and after operation, prognosis and mean drainage volume of the first three days after operation. Results: The operating time of LH group was longer than that of OH group. There were obvious differentiation between the two groups in operative bleeding volume, postoperative complication and prognosis. The time of postoperative food intake and days of hospitalization of LH group were shorter than that of OH group. The changes of serum levels of albumin and alanine aminotransferase in LH group before and after the operations were smaller in LH group than in OH group. Conclusions: LH has the merits of microinvasive and rapid recovery. It is safe and feasible if optimal patients were chosen.
