学龄期儿童潮气量与营养状态的相关性研究

摘    要:目的 研究学龄期(6~12岁)儿童体格发育情况与潮气量大小的关系。方法 随机选取延安宝塔区学龄期(6~12岁)儿童4279名,测量并记录其身高、体重、BMI、胸围、腹围、腹皮脂厚度和潮气量大小,并对身高、体重、BMI、胸围、腹围和腹皮脂厚度与潮气量大小的相关性进行分析。结果 在4279名儿童中,超重儿童397名(9.28%),肥胖儿童504名(11.78%);超重和肥胖儿童的潮气量比同年龄段儿童潮气量小,P<0.05,差异有统计学意义;身高、体重、体重指数、胸围、腹围与潮气量的大小存在相关关系,相关系数皆大于0.7,P<0.05,差异有统计学意义,且腹围与潮气量大小的相关性在数值上高于体重等其他营养状态指标;腹皮脂厚度对潮气量的影响不显著,P>0.05,差异无统计学意义。结论 学龄期(6~12岁)儿童潮气量的大小受营养状态指标如身高、体重、体重指数、胸围、腹围的影响,潮气量的大小不仅与体重呈正相关,更与胸围、腹围的大小密切相关;另外,本研究发现肥胖儿童潮气量低于正常儿童潮气量。因此在儿童机械通气设置参数时除考虑体重外应考虑胸围、腹围、身高、体重指数等营养状态指标对潮气量大小的影响。

Abstract:Objective To study the relationship between physical development and tidal volume of school-age children (ages 6~12).Methods 4279 school-age children (6~12 years old) from Baota district,Yan′an city were randomly selected to measure and record their height,weight,BMI,chest circumference,abdominal circumference,abdominal sebum thickness and tidal volume,and the correlation between height,weight,BMI,chest circumference,abdominal circumference and abdominal sebum thickness and tidal volume was analyzed.Results Among the 4279 children,397 were overweight (9.28%) and 504 were obese (11.78%).The tidal volume of overweight and obese children was smaller than that of children of the same age group(P<0.05),the difference was statistically significant.Height,weight,body mass index,chest circumference and abdominal circumference were correlated with tidal volume,and the correlation coefficients were all greater than 0.7 (P<0.05).The differences were statistically significant,and the correlation between abdominal circumference and tidal volume was higher than other nutritional status indicators such as body weight.The effect of abdominal sebum thickness on tidal volume was not significant,P>0.05,the difference was not statistically significant.Conclusion The tidal volume of school-age children (6~12 years old) is affected by nutritional status indicators such as height,weight,body mass index,chest circumference and abdominal circumference.The tidal volume is not only positively correlated with body weight,but also closely correlated with chest circumference and abdominal circumference.In addition,the tidal volume of obese children was lower than that of normal children.Therefore,the influence of nutritional status indexes such as chest circumference,abdominal circumference,height and body mass index on tidal volume should be considered in setting parameters of mechanical ventilation for children in addition to body weight.
