NBI-ME与白光内镜在早期胃癌及癌前病变诊断中的对比分析

摘    要:目的 探讨窄带成像放大内镜在早期胃癌及癌前病变诊断中的优越性及应用价值。方法 对榆林市第二医院2016年9月至2018年3月发现42例胃黏膜可疑病变应用NBI-ME与白光内镜检查进行对比分析。结果 经病理确诊早期胃癌5例,重度异型增生10例,轻中度异型增生18例,肠上皮化生9例。白光内镜漏诊10例,检出率76.19%;NBI-ME漏诊3例,检出率92.86%。NBI-ME对早期胃癌及癌前病变诊断的准确性显著高于白光内镜(P<0.05)。结论 NBI-ME对于胃黏膜微血管形态的显示具有独特的优势,对病变靶向活检具有一定的指导意义,能提高早期胃癌及癌前病变诊断的准确性。

Abstract:Objective To evaluate the advantages and value of narrowband imaging magnification endoscopy(NBI-ME) in the diagnosis of early gastric cancer and precancerous lesions.Methods NBI-ME and white light endoscopy were used to compare 42 cases of suspected lesions found in second Hospital of Yulin City from September 2016 to March 2018.Results 5 cases of early gastric cancer,10 cases of severe dysplasia,18 cases of mild and moderate dysplasia and 9 cases of intestinal metaplasia were diagnosed by pathology.10 cases were missed by white light endoscopy,the detection rate was 76.19%; NBI-ME was missed in 3 cases and the detection rate was 92.86%.The accuracy of NBI-ME in the diagnosis of early gastric cancer and precancerous lesions was significantly higher than that of white light endoscopy(P<0.05).Conclusion NBI-ME has a unique advantage in the display of gastric mucosal microvessel morphology,and has a certain guiding significance for lesion-targeted biopsy,which can improve the accuracy of early gastric cancer and precancerous lesion diagnosis.
