多层螺旋CT在急诊腹部损伤中的应用

目的:探讨MSCT对急诊腹部损伤检查的积极意义和早期准确诊断腹部脏器损伤的重要性。材料与方法:自2004年4月—2005年7月在MSCT下对16例急诊腹部损伤的病人进行了检查,年龄16~58岁,所用螺旋CT为GE公司生产的LIGHT SPEED多层螺旋机,从剑突下延伸到联合下缘扫描,传入Advantage Windows工作站利用MPR、SSD、3D显示腹部及脊椎各细节。结果:在16例急诊腹部损伤中发现:肝破裂5例、脾破裂6例、肾破裂4例、肝脏刀刺穿通伤伴小网膜及胃壁损伤1例。MSCT多方位观察利用MPR、SSD、3D检查方法均能得到二维、三维清晰图像。结论:对肝、脾、肾损伤程度提出评价,为临床医师提供准确的损伤部位、近似真实的三维立体图像,减少了患者X线受线量,避免了不必要的重复检查,避免了在复合性腹部损伤时因搬运而造成的人为损伤。

Appliance of MSCT in the Emergency of Abdominal Injury

Purpose:To discuss the positive significance and importance of MSCT in the early examination of abdominal emergency.Materials and Methods:From April 2004 to July 2005,16 cases of abdominal emergency,from 16 to 58 years old,were examined by MSCT,which were produced by GE Co.USA.They were scanned from xiphoid process to bublic symphysis and the message were delivered to the working station to show the details of abdomen and vertebrace by MPR,SSD and 3D.Results:In this group,5 cases of rupture of liver,6 cases of splenic rupture,4 cases of rupture of kidney,and 1 case of puncture of liver with the injure of lesser omentum and gastric wall were found,and MPR,SSD,3D could supply 2-dimensional or 3-dimensional images.Conclusion:MSCT could correcty provide 3-dimensional images and the detailed position of injure for the clinical doctors,reduce the X-ray dose for the patients and avoid the repeated examination and further injury while the patients being carried.
