适形放射治疗胰腺癌的疗效分析

目的探讨三维适形放射治疗胰腺癌的疗效及毒性.方法对21例胰腺癌患者采用三维适形放射治疗.单次剂量为2 Gy,每日照射一次,每周照射5次,总疗程6周左右,总剂量为60 Gy.结果临床获益率28.6%(6/21),疼痛完全缓解率90.5%(19/21).瘤体CR率为9.5%,PR率为52.4%,总有效率(CR PR)为61.9%(13/21).中位生存期15个月,1年生存率67.0%.治疗后出现血像下降Ⅰ～Ⅱ级者14例,Ⅲ级者1例,Ⅰ～Ⅱ级恶心、呕吐12例.结论三维适形放射治疗能够改善胰腺癌患者的生活质量,提高生存率.

3-dimensional conformal radiotherapy for pancreatic malignancies

Objective To assess the efficacy and toxicity of 3-dimensional conformal radiotherapy (3DCRT) for pancreatic malignancies.Methods Twenty-one patients with pancreatic cancer received 3DCRT ~60 Gy at 2 Gy/d,5 fractions per week for 6 weeks.Radiotherapy was given using the Stryker Leibinger Radio System and a 6-MV photon beam.Results The response rate was 61.9% (2 complete responses and eleven partial responses).Complete control of pain was 90.5%.Clinical benefit response was 28.6%.The median survival duration and the 1-year survival rate were 15 months and 67.0% respectively.The treatment was tolerated well.Grade-1/2 nausea/vomiting developed in 12 patients(57.2%) and Grade-1/2 leuxocytopenia developed in 14 patients (~66.7% ). Only 1 patient(4.8%) presented Grade-3 toxicity.Conclusion This study demonstrates that 3DCRT improves symptoms and survival in patients with pancreatic cancer.
