前方牵引治疗上颌骨发育不足——侧貌改变的回顾性研究

目的 探讨前方牵引结合快速扩弓骨性前牙反合的疗效。方法 上颌发育不足引起的骨性Ⅲ类错牙合患者18例。治疗前、中、后分别拍摄头颅侧位X线片,进行头影测量分析,统计学分析采用方差分析。结果 所有患者均由Class Ⅲ矫正为Class I类,面型得到了明显的提高和改善,骨骼和软组织以及唇的位置均得到了矫正。Class III关系有复发趋势,可能是下颌生长导致。结论 上颌快速扩弓结合前方牵引软组织面型得到了明显的提高和改善,骨骼和软组织以及唇的位置均得到了良好的矫正,很大程度上减小了患者成人后进行正颌手术的可能性。上颌骨前移的效果基本是稳定的,Class Ⅲ的复发趋势主要是由于下颌骨进一步生长的结果。

Retrospective analysis of maxillary protraction in the treatment of profile change caused by maxillary deflciency

Objective To investigate the curative effect of maxillary protraction combined with rapid maxillary expansion in the treatment of skeletal anterior crossbite. Methods There were 18 cases of Class HI skeletal crossbite caused by maxillary deficiency. The lateral eephalogTam was taken before the treatment, during the treatment and after the treatment. Cephalometrics analysis and analysis of variance were used. Results All patients were rectified from Class m to Class I, and their facial forms were improved. The skeleton, soft tissue and the posture of lips were all improved. Because of mandibular growth, the Class Ⅲ correction was tended to loss. Conclusion Maxillary protraction combined with rapid maxillary expansion provides obvious improvement on soft tissue facial profiles. The skeleton, soft tissue and the posture of lips were rectified. It reduces the probability for the patients undergoing orthognathic surgery in the future. The effects of maxillary protraction appear to be stable. The reappearance of Class Ⅲ pattern is primarily related to mandibular growth.
