支气管典型类癌的CT表现及其诊断价值

目的探讨支气管典型类癌（BTC）的CT表现及诊断价值。方法回顾性分析我院经手术病理证实的5例BTC，结合文献分析其CT征象。结果5例BTC位于右肺3例，左肺2例；3例中央型，2例周围型；直径3～5cm，圆形2例，椭圆形2例，该4例表面光滑无分叶，1例有多个浅分叶；1例肿块外侧见柱状支气管囊肿，边缘清晰锐利，其余4例未见肺内伴随病变；5例病灶密度均匀，其中，2例CT值低于心脏大血管，增强扫描3例均呈中度均匀强化。5例病灶均未见液化、钙化及纵隔淋巴结肿大。结论CT扫描有助于部分具有典型征象BTC的诊断，但仍需与支气管原性肺癌鉴别。

CT findings of bronchial typical carcinoid and their significance for the diagnosis

Objective To analyze the CT findings of bronchial typical carcinoid(BTC) and to assess their significance for the diagnosis.Methods CT findings of 5 patients with surgically and pathologically proved BTCs were retrospectively analyzed together with review of the literature.Results CT demonstrated that lesions of BTCs were located in right lung(n=3) and left lung(n=2) ,respectively.There were 3 central BTCs and 2 peripheral BTCs in 5 cases.The size of BTCs in all patients ranged from 3 cm to 5 cm in diameter.BTCs in 4 cases presented round(n=2) and oval(n=2) with sharp surface but without lobulations,while that in 1 case had many shoal lobulations.The bronchial colum-from cyst with well margin was found in 1 case,whereas other 4 cases no any intrapulmonary complications were found.The density of BTCs in all patients was homogeneous but 2 cases whose CT value of BTCs were lower than that of cardiac large vessels.After enhanced scans,BTCs in 3 cases appeared middle homogencous enhancement.In all patients,no liquefaction,calcification of BTCs and enlargement of mediastinal lymphonodes were observed.Conclusion CT scanning may help to diagnose partial patients with clinically suspected BTC but the definite diagnosis needs to differentiate from bronchogenic lung cancer.
