超声造影检测肝血流动力学改变在诊断肝转移癌中的价值

摘    要:目的探讨超声造影检测肝血流动力学改变在诊断肝转移癌中的价值。方法经肘静脉团注声诺维造影剂(0.6ml)后利用时间一强度分析软件对41例肝转移癌患者(肝转移癌组)和18例健康志愿者(正常对照组)的肝动脉到达时间、肝动脉峰值时间、肝动脉峰值强度、肝静脉到达时间、肝静脉峰值时间和肝静脉峰值强度进行测定,并计算出肝动静脉渡越时间。结果肝转移癌组的肝动脉到达时间、肝动脉峰值时间、肝静脉到达时间和肝静脉峰值时间均较正常对照组提前(P〈0.01),肝动脉峰值强度和肝静脉峰值强度高于正常对照组(P〈0.01),肝动静脉渡越时间较正常对照组缩短(P〈0.001)。结论超声造影检测肝血流动力学改变有助于肝转移癌的诊断。

Abstract:Objective To investigate the value of detecting the changes of hepatic haemodynamics in diagnosis of liver metastasis by contrast enhanced ultrasound.Methods Forty-one patients with proven liver metastases(study group) and 18 normal volunteers(control group) had been recruited to be studied.Each subject was administrated with a bolus injection of the ultrasound microbubble agent SonoVue((0.6) ml) into an arm vein.The arrival times in the hepatic artery(AT_(HA)),time to peak in the hepatic artery(TTP_(HA)),peak intensity of the hepatic artery(PI_(HA)),arrival times in the hepatic vein(AT_(HV)),time to peak in the hepatic vein(TTP_(HV)) and peak intensity of the hepatic vein(PI_(HV)) were measured with the use of time-intensity curve software.The hepatic artery to vein transit time(HAVTT) was calculated as the difference between the arrival times in the hepatic artery and the hepatic vein.Results AT_(HA),TTP_(HA),AT_(HV) and TTP_(HV) in study group were significantly shorter than those of control group(P<(0.01)).PI_(HA) and PI_(HV) in study group were significantly brighter than those of control group(P<(0.01)).HAVTT was significantly in study group shorter than that of control group(P<(0.001)).Conclusions Detecting the changes of hepatic haemodynamics by contrast enhanced ultrasound help to diagnose liver metastases.
