经阴道彩色多普勒超声对卵巢滤泡囊肿的诊断

目的探讨卵巢囊肿经阴道彩色多普勒超声检查的声像图特点,评价经阴道彩色多普勒超声检查对诊断卵巢滤泡囊肿的诊断价值。方法对858例拟诊为卵巢滤泡囊肿的患者行经阴道彩色多普勒超声检查,根据卵巢滤泡囊肿的超声声像图特点进行诊断,确诊者追踪观察6～8周,根据卵巢滤泡囊肿的病程特点及变化过程,判断诊断的符合率。结果 858例患者均于阴道彩色多普勒超声检查治疗6～8周后随访,其中826例患者囊肿消退,10例原囊性包块仍然存在（8例经手术病理证实为浆液性囊腺瘤,2例囊肿直径〈2 cm）。其中20例另侧卵巢再次出现囊性包块,根据其声像图特点仍诊断为滤泡囊肿,继续随访6～8周,19例囊肿消退,1例囊肿直径〈2 cm。2例发生滤泡囊肿破裂导致急腹症接受手术治疗。卵巢滤泡囊肿经阴道彩色多普勒超声诊断符合率为99.0%。结论根据滤泡囊肿的经阴道彩色多普勒超声声像图特点对患者进行追踪观察,可为临床制订个体化的治疗方案提供依据。

Diagnosis of Ovarian Follicular Cysts with Transvaginal Color Doppler Ultrasound

Objective To explore the ultrasonographic features of ovarian follicular cysts and to evaluate the value of transvaginal color doppler ultrasound in diagnosing ovarian follicular cysts.Methods Transvaginal color Doppler ultrasound was performed in 858 patients with suspected ovarian follicular cysts.The diagnosis was made according to the ultrasonographic features.Patients with confirmed ovarian follicular cysts were observed for 6-8 weeks.The coincidence rate of the diagnosis was determined according to course characteristics and change process of ovarian follicular cysts.Results Among the 858 patients transvaginal color doppler ultrasound who were followed up after treatment for 6-8 weeks,cysts subsided and no obvious cysts existed in contralateral ovary in 826,original cystic mass still existed in 10（8 patients with serous cystadenoma confirmed by pathology report and 2 patients with cysts less than 2 cm in diameter）,follicular cysts confirmed by ultrasound recurred in contralateral ovary in 20（cysts subsided in 19 and cyst diameter was less than 2 cm in 1 after 6-8 weeks of additional follow-up）,and operative treatment was performed in 2 due to acute abdomen following cyst rupture.The diagnostic accordance rate of transvaginal color Doppler ultrasound for ovarian follicular cysts was 99.0%.Conclusion The follow-up observation according to transvaginal Doppler ultrasound features can provide a basis for the clinical development of individualized treatment program in patients with ovarian follicular cysts.
