多层螺旋CT对局灶性炎症样周围型小肺癌的诊断

目的探讨局灶性炎症样周围小肺癌的多层螺旋CT影像特征。方法回顾性分析经手术病理证实的35例局灶性炎症样周围型小肺癌的多层螺旋CT表现,分析内容包括在2 mm薄层肺窗图上病变的大小、形态、密度、毛刺征、细支气管充气征、空泡征、血管集束征、胸膜凹陷征。结果 35例病变直径均小于2 cm,35例均形态不规则、密度浅淡,细短毛刺征23例,僵硬细支气管充气征30例,空泡征29例,血管集束征31例,胸膜凹陷征23例。结论局灶性炎症样周围型小肺癌具有边缘欠规整,有小空泡、细短毛刺、僵硬的细支气管充气征、血管集束征及胸膜凹陷征等多层螺旋CT征象,这些征象有助于其定性诊断。

Multi-slice Spiral CT in Diagnosis of Focal Pneumonia-like Small Peripheral Lung Cancer

Objective To investigate the multi-slice spiral CT features of focal pneumonia-like small peripheral lung cancer.Methods The multi-slice spiral CT findings of 35 cases of pathologically proven focal pneumonia-like small peripheral lung cancer were analyzed retrospectively,including size,shape,density,coarse spicule sign,air bronchogram sign,vacuole sign,vessel convergence sign and pleural indentation sign of lesions on the 2 mm thin layer of lung window image.Results All lesions had diameters less than 2 cm with irregular shape and pale density.Of the 35 cases,short coarse spicule sign was shown in 23,stiff air bronchogram sign in 30,vacuole sign in 29,vessel convergence sign in 31,and pleural indentation sign in 23.Conclusion The multi-slice spiral CT findings of focal pneumonia-like small peripheral lung cancer include irregular edge,vacuole sign,short coarse spicule sign,stiff air bronchogram sign,vessel convergence sign and pleural indentation sign.These features can contribute to the qualitative diagnosis.
