颅内多发动脉瘤的外科治疗(附16例报告)

目的：探讨颅内多发动脉瘤的发生率、治疗原则和预后。方法：颅内多发动脉瘤16例，其中15例有2个动脉瘤，1例有3个动脉瘤。33个动脉瘤中，颈内一后交通动脉瘤占75．8％，眼动脉瘤占12.1％，前交通动脉瘤和大脑中动脉动脉瘤分别占9．1％和3．0％。一期手术夹闭所有动脉瘤12例，分期手术夹闭2个动脉瘤1例，一侧瘤颈结扎或包裹十另一侧顿外颈动脉结扎2例，另1例仅处理一个动脉瘤。结果：优良15例，轻残1例。结论：尽可能一期手术处理所有动脉瘤，预后良好。

Surgical Treatment of Multiple Intracranial Aneurysms(A Report of 16 Cases)

Objectives: To explore the incidence,principles of treatment and the prognosis of multiple intracranial aneurysms (MIA). Methods:Of the 16 cases with MIA, 15 had two aneurysms and 1 hadthree aneurysms. Among 33 aneurysms, 25 (75. 8% ) were located in the carotid-posterior communicating artery, 4 (12. 1 % ) in the ophthalmic artery, 3 (9. 1 % ) in the anterior communicating artery,and 1(3. 0% ) in the middle cerebral artery. All presented with subarachnoidal haemorrhage (SAH). Onestage operations were performed in 14 cases,two-stage operation in 1 and just one aneurysm was operated upon in the remaining 1 case. Results: Recovery was excellent or good in 15 cases,mild disability in1 case. Conclusions: Prognosis is good for multiple aneurysms treated in one-stage operations and thestrategy should be applied whenever possible.
