MRI诊断脊柱结核38例分析

目的探讨脊柱结核的MRI表现及其诊断价值。方法对38例经手术病理证实的脊柱结核患者的MRI资料进行回顾性分析。结果38例脊柱结核，共111个椎体受累，13例椎体变形，33例椎闻盘受累，表现为椎间隙消失7例，椎间隙狭窄24例，2例椎间隙未见改变，仅表现为信号异常；37例见冷脓肿影，硬膜囊及脊髓受压6例，4例于T2WI及T2压脂可见脊髓异常高信号。结论MRI可清楚早期显示脊柱结核椎体破坏，椎间隙改变，椎间盘信号改变，椎旁脓肿范围，硬膜囊及脊髓受压情况，对临床早期诊断及指导治疗有重要意义。

Diagnosis of Sspinal tuberculosis with MRI

Objective To investigate MRI appearances of spinal tuberculosis and the value of MRI in diagnosis of spinal tuberculosis. Methods To retrospectively study MRI exams of 38 postoperated patients with spinal tuberculosis histopathologically proved. Results There were 111 vertebral bodies involved in 38 patients, including 13 cases with deformation, 33 cases with intervertebral disks involved, 24 cases with intervertebral space narrowed, 7 cases with intervertebral space disappeared, 37 cases with cold abscess and 6 cases with dural sac and spinal cords pressed. Conclusion MRI shows the destruction of vertebral body, the changes of the intervertebral space, the signal changes of intervertebral disk and the scope of the paraspinal abscess of spinal tuberculosis, which is helpful for early clinical diagnosis and treatment for spinal tuberculosis.
