预置可拆膀胱颈荷包缝合法在耻骨上前列腺摘除术的应用——100例临床观察

作者在耻骨上前列腺摘除术中将现今常用的膀胱颈荷包缝合止血法改进为“预置可折膀胱颈荷包缝合法”,即在摘除腺体前预置单一膀胱颈荷包缝合线,腺体摘除后将缝合线圈绕导尿管以鞋带式活结结扎于膀胱外,借以隔离腺窝以达到止血的目的,又便于术后拆除,使膀胱颈和尿道内口自然复位,腔内无异物。作者自1979年元月起采用此法行耻骨上前列腺摘除术,经连续100例观察,效果较为满意。

Application of a Removable Purse-string Suture Prefabricated on the Bladder Neck to Suprapubic Prostatectomy——A Clinical Observation of 100 Cases

The technique of a removable purse-string suture prefabricated on the bladder neck, which has greatly improved the traditional hemostasis method with pursestring closure on the bladder neck, has been applied to 100 patients who underwent suprapubic prostatectomy since 1979. A singular pursestring suture prefabricated on the bladder neck before prostatectomy is knoted around the catheter in the form of shoelace slipknot outside the bladder following prostate resection. This makes it easier to isolate the bladder from the prostate fossa to stop bleeding and remove the suture postoperatively to facilitate the natural reposition of the bladder neck and urethral orifice, while no foreign bodies remain in the cavity. It has been demonstrated that this technique has an advantage over the traditional hemostasis because of its simple procedure, shortened operating time, less bleeding during the operation, and less postoperative complications.
