青少年甲亢131I治疗的临床初探

摘    要:目的:探讨青少年甲亢131I治疗的临床疗效、副作用及治疗后病人满意度情况等问题。方法:对89例14~25岁(甲状腺Ⅱ度肿大以上)的青少年甲亢采用131I治疗后随访3年以上的资料进行分析,并利用信访或面谈等形式了解患者对该治疗方法的满意度情况。结果:89例患者中,一次治愈61例(占68.5%),好转14例(占15.7%),甲低(永久性)11例(占12.4%),无效3例(占3.4%);从已随访到的情况看,所有患者治疗后无发生甲状腺结节、甲状腺癌变、其它癌或白血病病例,26例已婚生育者后代中无致畸现象发生;131I治疗后,对该治疗方法81例(91%)认为满意,5例(5.6%)认为一般,不满意者3例(3.4%)。结论: 131I是治疗青少年甲亢有效、经济、相对安全的方法,患者对该治疗方法满意度高,对甲状腺肿大明显(Ⅱ度以上)、病程长的青少年甲亢患者,131I治疗是值得推荐的方法。

Abstract:Objective:To probe into the clinical curative effect, side effect and gratification degree of adolescent hyperthyroidism after 131I therapy. Methods: To analyse the material of 89 cases (age range from 14-25 ) treated with 131I for Grave's disease over 3 years follow-up and the patients gratification degree to the treatment was known with mail or interview. Results: 61 cases (68.5%) were once cured among the 89 patients, improvement in 14 cases (15.7%), hypothyroidism in 11 cases(12.4%), ineffective in 3 cases (3.4%). There were no thyroid nodules, thyroid cancer ,other malignancies or leukemia in all cases after treatment, neither were teratogenesis in progeny of 26 legitimate. After 131I therapy, 91% of patients were satisfied, 5.6% felt it common, only 3.4% dissatisfied. Conclusion: 131I is found to be an effective, economic and safe treatment of Graves' disease in adolescents ( especially to the obviously goitrous patients) , the treated patients have high gratification and it should be the preferred mode of therapy.
