腹腔镜下行卵巢肿瘤剥除术476例报告

目的 探讨腹腔镜下行卵巢肿瘤剥除术的临床应用价值.方法 476例术前诊断为卵巢良性肿瘤的患者在腹腔镜下行剥除术,并保留其正常的卵巢组织.结果 476例均成功剥除,3例术中剥除组织送冷冻病理组织检查提示恶性肿瘤而中转开腹,2例术后病理检查提示恶性肿瘤而行第2次开腹手术,无1例发生术中脏器损伤、术中大出血、皮下气肿、术后化学性腹膜炎等并发症,平均手术时间85 min,术中平均出血量50 mL,术后8～24 h排气,1、2 d起床活动,2 d恢复正常饮食,3～7 d出院.结论 严格掌握手术适应证,术前术中准确判断,腹腔镜下行卵巢肿瘤剥除术,出血少,损伤小,并发症少,安全有效,优于常规开腹手术.

476 cases of laparoscopic ovarioncas operation

[Objective] To study the clinical value of laparoscopic operation in oophorocystectomy. [Methods] Four hundred and seventy-six ovarial innocent tumor cases were diagnosed before operation and treated by laparoscope that exciser ovarioncus but reserve its normal ovary organization. [Results] In these patients, all of 476 cases were succeed. Among them, 3 cases changed to laparotomy in the operation for it's frozen section clue to therioma, and 2 cases had the second-time laparotomy for the pathologic diagnosis of postoperative clue to therioma. No complications took place: organoid injury in the operation, hematorrhea, aerodermectasia, chemical peritonitis after operation. The average surgical operatioly was time 85 mins, and average hemorrhage was 50 mL. Air was out 8~24 hour after operation, patients could move after 1~2 days, and 2 d recoverd eating, and 3~7 fully recover. [Conclusion] We should control the indication strictly, diagnose accurately, and adopt ovarioncus by laparoscope. It is with few hemorrhage, scatheless, oligo-complications, safety and availability. It is better than the normal celiotomy.
