副肿瘤性小脑变性与抗神经元抗体的关系

摘    要:报告17例副肿瘤性小脑变性患者,多为中老年发病,急性或亚急性起病,表现眩晕、肢体和小脑共济失调等症状,疾病进行性发展,数周或数月内完全丧失自理能力。88.3%的患者先有小脑或神经系统症状,经详细检查后发现原发肿瘤,肿瘤以肺癌、乳腺癌和卵巢癌多见。血清和脑脊液中抗神经元抗体(抗Yo、抗Hu、抗Ri)阳性率为83.3%和45.5%,血清肿瘤标志物检测阳性率31.3%,CT、PET或活检、手术病理检查是主要确诊手段。对急性或亚急性起病的眩晕、共济失调患者,应检测抗神经元抗体,抗体阳性者全面进行肿瘤筛查,特别是乳腺、盆腔和肺部等部位检查,CT、PET或PET/CT一体机检查具有重要价值。

Abstract:17 patients with paraneoplastic cerebellar degeneration (PCD) were selected analyzed. Most patients presented with acute and subacute vertigo and cerebellar ataxia. The disease progressed very rapidly and lost self- care ability in several weeks or months. The most commonly associated tumours were lung cancer, breast and gynaecological cancer. Neurological symptoms preceded the diagnosis of primary turnouts in 88.3% patients. Antineuronal antibodies (anti-Hu, Yo, Ri) were detected positively in serum (83.3 %) and cerebral spinal fluid (45.5 %). The examination of antineuronal anti- bodies is necessary for the patients suffered from acute or subacute vertigo and cerebellar ataxia. If the antibodies are positive, it is advocated an intensive investigation on underlying tumour such as lung, breast and gynaecological cancers. CT, PET or PET/CT imaging is very useful in diagnosis of tumours.
