| 摘 要： | 目的探讨后程适形放疗联合同期化疗对中上段食管癌的近期疗效。方法对61例中上段食管癌患者随机分为两组,同期放疗、化疗组(放化组)和单纯放疗组(单放组),均行前程普通放疗加后程适形放疗。前程普通放疗行三野等中心照射,剂量DT40Gy/20次,4周完成;后程适形放疗剂量DT26~30Gy/13~15次,2.5~3周完成。总剂量DT66~70Gy,33~35分次,6.5~7周完成。放化组在放疗开始即行DF方案(顺铂+氟尿嘧啶)化疗,共2周期。结果放化组和单放组1年有效率分别为83.9%(26/31)和63.3%(19/30),放化组发生放射性食管炎、WBC值下降等高于单放组。结论与对照组相比,后程适形放疗同步化疗治疗食管癌明显提高了患者的近期疗效及局部控制率,且正常组织的放射性损伤未明显增加。
