肋间神经冷冻术预防术后疼痛临床应用研究

摘    要:目的:探索应用冷冻治疗肋间神经痛的疗效和方法。方法:将120例需开胸手术的患者随机分成两组,冷冻组与对照组各60例,冷冻组手术结束前冷冻切口、上下各一肋间以及放置引流管之肋间神经根部,根据视觉模拟评分法观察术后切口疼痛以及止痛药使用情况,对照组不用冷冻治疗。结果:冷冻组止痛效果极为满意,VAS评分为2.2,而对照组为7.5,差异有显著性意义(P〈0.01);冷冻组总有效率为97%,吗啡平均用量为2.5mg;而对照组总有效率56%,吗啡平均用量为15.2mg,两组差异有显著性意义(P〈0.01)。结论:冷冻肋间神经能够有效地预防开胸术后剧烈胸痛。

Prevention of pain after thoracotomy by freezing intercostals nerves

Abstract:Objective To explore the effect and the method of treating the pain after thoracotomy by freezing intercostals nerves. Method For 120 patients undergoing thoracotomy were randomly divided into two groups as the frozen group and the contrast group. Each group had 60 patients. The patients in the frozen group were froze the intercostals nerves of operation wound, the intercostals nerves adjacent operation wound, and the intercostals nerves which intercostals space placed the drainage tube. The clinic effects were assessed by visual analogue pain scale (VAS) and the circumstance of morphine using after the operation. The patients in the contast group were not froze the intercostals nerves. Results The VAS score of pain in the frozen group was 2.2, and the contrast group was 7.5. Compare to the contrast group, the VAS score of pain in the frozen group was significantly lower (P<0.01). The effectiveness of the frozen group was 97%, and the average dosage of morphine was 2.5mg. The effectiveness of the contrast group was 56%, and the average dosage of mouphine was 15.2mg. The difference between the two groups was significant (P<0.01). Conclusion The treatment of freezing intercostals nerves is effective to cure the pain after thoracotomy.
