显微神经外科手术治疗听神经鞘瘤22例体会

目的 探讨听神经鞘瘤的显微手术技术与疗效.方法 回顾性分析我院2006年6月至2012年6月间经枕下乙状窦后入路显微手术切除22例听神经鞘瘤患者临床资料.结果 术中听神经鞘瘤全切17例,次全切5例,面神经解剖保留21例,面神经功能保留17例,死亡0例,术后脑脊液漏1例,再出血1例.结论 采用枕下乙状窦后入路显微手术切除听神经鞘瘤可取得满意的肿瘤切除和神经保护的效果.

Experience of microsurgery of acoustic neuromas(report of 22 cases)

Objective To evaluate the surgical technique and ettect of mlcrosurgery tor acoustlc neuromas. Methods The clinical date of 22 patients with acoustic neuromas treated via suboccipital retrosigmoidal approach from June 2006 to June 2012 in our hospital were analyzed retrospectively. Results 17 cases of acoustic neuromas were resected totally. 5 cases were resected subtotally. Faci~d never was preserved in 21 cases anatomically and in 17 cases functionally. No surgical death was reported. Postoperative. cerebrospinal fluid leakage occurred in 1 case and postoperative bleeding 1 case. Conclusion Microsurgery via suboecipital retrosignoid approach can get high rate of total acoustic neuromas removal and preservation of facial nerve.
