奥布卡因凝胶联合心理干预在特殊人群置器效应分析

目的探讨奥布卡因凝胶联合心理干预在特殊人群（宫口无扩张史的剖宫产者，宫颈经微波、LEEP等手术治疗者）放置宫内节育器的临床效应。方法将126例特殊人群分成两组，观察组放置宫内节育器术中加用奥布卡因凝胶，并辅以心理干预，对照组常规行宫内节育器放置术。观察两组宫颈软化、疼痛程度、手术时间、心脑综合征、手术满意度差异。结果观察组总有效率高于对照组，差异有统计学意义（x2=50．958，P〈0．01），而手术时间较对照组缩短（t=10．572，P〈0．001），疼痛评分低于对照组（t=27．284，P〈0．001），心脑综合征发生率低于对照组（x2=8．818，P〈0．05）。观察组手术满意度95．31％，对照组手术满意度67．74％，两组比较差异有统计学意义（x2=16．04，P〈0．01）。结论奥布卡因凝胶联合心理干预应用于特殊人群置宫内节育器术中，能有利于手术操作，缩短手术时间，减轻疼痛，提高患者满意度。

Effect of oxybuprocaine gel combined with psychological intervention on special population in the operation of putting IUD

Objective To investigate the clinical effect of oxybuproeaine gel combined with psychological intervention on special population when putting intrauterine device （IUD） （women with history of cesarean delivery but without previous record of cervical dilations, women with cervical treatment by microwave or LEEP operations）. Methods Totally 126 cases of special population were divided into observation group and control group. The observation group placed IUD with oxybuproeaine gel and psychological intervention, while the control group placed IUD with conventional method. The differences in cervical softening, pain degree, operation time, eardio-eerebral syndrome and operation satisfaction were observed. Results Total effective rate of the observation group was significantly higher than that of the control group （X2 =50. 958, P 〈0.01）. The operation time was significantly shorter （t = 10. 572, P 〈0. 001 ）, the pain score was lower （t = 27. 284,P 〈0. 001 ）, and the incidence of cardio-cerebral syndrome was significantly lower （X2 = 8. 818, P 〈0.05） in the observation group than the control group. The operation satisfaction degree of the observation group was 95. 31% , which was higher than that （67.74%） of the control group, and the difference was significant （X2 = 16.04 ,P 〈0.01 ）. Conclusion Oxybuprocaine gel combined with psychological intervention is helpful to shorten operation time, relieve pain and improve patients＇ satisfaction degree for the special population at the course of taking IUD.
