羊膜移植治疗儿童眼外伤的临床研究

目的探讨羊膜移植治疗儿童眼外伤引起的眼角膜损伤的疗效，为儿童眼外伤患者提供一种新的治疗方法。方法对2007年9月至2013年8月五河县人民医院眼科收治的58例67眼外伤性角膜损伤或穿孔的患者采用多层羊膜覆盖治疗，术后随访6个月，观察角膜愈合情况、羊膜开始溶解时间及术后视力变化等情况。结果术后1天58例患者前房均形成良好。术后1个月全部角膜完成修复，其中36只眼角膜透明，16只眼遗留云翳，13只眼遗留斑翳，2只眼遗留白斑。术后1周羊膜开始溶解。术后随访6个月65只眼的视力均有不同程度的提高。结论多层羊膜覆盖是治疗角膜穿通伤安全有效的方法，其能维持眼球结构的完整，减轻了炎症反应，为后续的穿透性角膜移植创造了良好条件。

Clinical study of.amniotic membrane transplantation for ocular trauma in children

Objective To investigate the clinical efficiency of amniotic membrane transplantation for cornea trauma caused by ocular trauma in children, so as to provide a new therapy for children with ocular trauma. Methods From September 2007 to August 2013, 58 patients having 67 eyes with cornea trauma received amniotic membrane transplantation in Wuhe People＇ s Hospital. All the patients were followed up for 6 months after operation, and the clinical efficacy, including repair of the cornea, amniotic membrane starting to dissolve, and postoperative visual acuity were carefully observed. Results All of 58 patients had the integrity of anterior chamber successfully one day after operation. After one month of the surgery, all corneal area healed in all cases, including 36 eyes having corneal transparency, 16 eyes with a different degree of caligo corneae, 13 eyes with corneal macula and 2 eyes with keratoleukoma, respectively. The amniotie membrane covering the surface of cornea started to dissolve one week after the surgery. The improvement of vision was found for certain degrees in 65 eyes after surgery by regular follow-up of 6 months. Conclusion Multiple amniotic membrane cover is an effective and safe method for the treatment of children＇ s corneal penetrating wound, as it can sustain the integrity of the eye structure, relieve the inflammatory reaction and create favorable conditions for later keratoplasty.
