带自体角膜缘上皮细胞的球结膜转位术防止进行性翼状胬肉的术后复发

目的　探讨带自体角膜缘上皮细胞的球结膜转位术和结膜瓣转位术治疗进行性翼状胬肉的疗效。方法　对114例进行性翼状胬肉患者随机分组 ,翼状胬肉切除后 ,甲组采用带自体角膜缘上皮细胞的球结膜转位术 ,乙组采用结膜瓣转位术进行治疗 ,术后随访 10～ 2 4个月 ,比较两组角膜创面修复时间、角膜新生血管、复发率。结果　术后角膜创面修复时间甲组较乙组短 (P <0 0 1) ,角膜新生血管两组差异无显著性 (P >0 0 5 ) ,复发率甲组较乙组低 (术后一年 ,P<0 0 5 ;术后两年 ,P <0 0 5 )。结论　带自体角膜缘上皮细胞的球结膜转位术为病变区角膜缘提供新的干细胞来源 ,治疗进行性翼状胬肉疗效好。

Protection of Conjunctival Flap Displacement with Limbal Epithelial Autograft Transplantation against Postoperative Recurrence of Progressive Pterygium

Objective To compare the therapeutic effect of conjunctival flap displacement with limbal epithelial autograft transplantation on the treatment of progressive pterygium. Methods 114 cases (114 eyes) of patients with progressive pterygium were randomly divided into 2 groups. After resecting pterygium under operating microscope, conjunctival flap displacement with limbal epithelial autograft transplantation and conjunctival flap displacement were performed on the first group and the second group, respectively. Corneal epithelial healing, corneal neovascularization and recurrent rate of pterygium were observed during the postoperative follow-up ranged from 10 to 24 months. Results Corneal epithelial healed faster in the first group than in the second group (P<0.01). Corneal neovascularization was not significantly different between the two groups(P>0.05). Pterygium recurrent rate of the first group was lower than that of the second group (12 months after operation, P<0.05, 24 months after operation, P<0.05). Conclusion Conjunctival flap displacement with limbal epithelial autograft transplantation can provide new stem cells source for defective limbus and has a better effect on the treatment of progressive pterygium.
