白细胞介素-37在活动性肺结核中对单核细胞分泌炎症因子的调节作用

摘    要:目的研究白细胞介素(interleukin,IL)-37、干扰素(interferon,IFN)-γ和IL-10在活动性肺结核患者单核细胞中的表达水平,探讨IL-37对IFN-γ和IL-10的调节作用及其临床意义。方法选取深圳龙岗中心医院2016年9月—2017年4月诊断为活动性肺结核的患者32例为结核组,以同期32例健康人群为对照组,提取外周血单核细胞,经脂多糖(LPS)刺激,IL-37抗体处理后,通过酶联免疫吸附法(ELISA)检测上清中IL-37、IFN-γ和IL-10水平,实时荧光定量PCR检测IL-10、IFN-γmRNA表达水平。结果活动性肺结核组上清中IL-37、IL-10水平较健康对照组升高(311.49±59.72) pg/mL vs (138.72±42.16) pg/mL、(302.65±38.2) pg/mL vs (65.86±6.06) pg/mL],IFN-γ水平下降(160.88±11.61) pg/mL vs (200.26±13.84) pg/mL],差异有统计学意义(P<0.05)。经IL-37抗体干预后,活动性肺结核组IFN-γ、IFN-γmRNA较干预前升高(193.76±12.46) pg/mL vs (160.88±11.61) pg/mL、(0.88±0.11) pg/mL vs (0.23±0.08)pg/mL],IL-10、IL-10mRNA较干预前下降(160.24±16.76) pg/mL vs (302.65±38.2) pg/mL、(8.32±2.82)pg/mL vs (15.84±4.02)pg/mL],差异有统计学意义(P<0.05)。健康对照组与干预前比较,IFN-γ、IL-10蛋白及mRNA表达差异无统计学意义(P>0.05)。结论活动性肺结核患者外周血单核细胞高表达IL-37;IL-37可抑制IFN-γ分泌,同时促进IL-10分泌,IL-37可能成为结核治疗的新靶点。

Abstract:Objective To study the expression of IL-37, IFN-γ and IL-10 in monocytes of patients with active pulmonary tuberculosis, and to explore the regulatory effect of IL-37 on IFN-γ and IL-10 and its clinical significance. Methods Select 32 patients with diagnosis of active tuberculosis in Longgang District Center Hospital of Shenzhen between September 2016 and April 2017 for the TB group, while 32 cases of healthy people as the control group at the same time. Peripheral blood monocytes were extracted, stimulated by lipopolysaccharide (LPS) and treated with IL-37 antibody. The levels of IL-37, IFN-γ and IL-10 in the supernatant were detected by enzyme-linked immunosorbent assay (ELISA) and real-time fluorescence quantitative PCR was used to detect IL-10, IFN-γ mRNA expression level. Results The serum IL-37 and IL-10 levels were higher in the patients with active pulmonary tuberculosis than in the control group (311.49±59.72) pg/mL vs (138.72±42.16) pg/mL, (302.65±38.2) pg/mL vs (65.86±6.06) pg/mL], and the IFN-γ level decreased (160.88±11.61) pg/mL vs (200.26±13.84) pg/mL], and the differenceswere significant (all, P<0.05). After IL-37 antibody intervention, IFN- γ and IFN- γ mRNA in active pulmonary tuberculosis group were higher than those before intervention(193.76±12.46) pg/mL vs (160.88±11.61) pg/mL, (0.88±0.11) pg/mL vs (0.23±0.08)pg/mL], IL-10, IL-10 mRNA was lower than before intervention (160.24±16.76) pg/mL vs (302.65±38.2) pg/mL,(8.32±2.82)pg/mL vs (15.84±4.02)pg/mL], and had significant significance (all, P<0.05). However, there was no significant difference in the expression of IFN-γ, IL-10 protein and mRNA between the healthy control group and the control group (P>0.05). Conclusion Peripheral blood monocytes overexpression IL-37 in the patients with active pulmonary tuberculosis; IL-37 can inhibit the secretion of IFN-γ and promote the secretion of IL-10, IL-37 may become a new target for tuberculosis therapy.
