结核病患者外周血和胸水T细胞中CD161表达差异的研究

摘    要:目的检测结核病患者外周血(PBMC)和胸水(PFMC)T细胞CD161水平,比较PBMC和PFMC T细胞CD161表达水平的差异,明确结核病人外周及感染部位T细胞CD161表达的差异。方法以流式细胞仪检测51例健康对照者、41例潜伏性结核感染患者、57例肺结核患者PBMC CD3^+CD161^+、CD4^+CD161^+和CD8^+CD161^+细胞的表达水平,比较其在不同人群中的差异;检测21例结核性胸膜炎患者PBMC和PFMC CD3^+CD161^+、CD4^+CD161^+和CD8^+CD161^+细胞的表达水平和差异,比较CD161在外周和感染局部的表达差异。结果利用流式细胞仪检测CD161在PBMC和PFMC中表达,并且分别在T细胞(CD3^+、CD4^+和CD8^+细胞)中表达;结核患者PBMC CD3^+CD161^+、CD4^+CD161^+和CD8^+CD161^+细胞表达均低于健康对照和潜伏感染者;结核性胸膜炎患者PFMC中CD3^+CD161^+和CD4^+CD161^+细胞表达均高于PBMC(P<0.01),但在PFMC中CD8^+CD161^+细胞则高于在PBMC中的表达(P<0.01)。结论 CD161均在PBMC和PFMC中表达,并且CD161能区分结核患者、潜伏感染者以及健康人。另外CD3^+CD161^+和CD4^+CD161^+细胞表达在结核病感染局部增加,结果提示CD161可作为早期诊断结核患者的分子标识。

Abstract:Objective Flow cytometry was used to detect CD161 levels in peripheral blood (PBMC) and pleural effusion cells (PFMC). The differences of CD161 expression levels in T cells between PBMC and PFMC were compared, to clarify the difference of CD161 expression in T cells of peripheral and local infection of tuberculosis patients. Methods The expression levels of PBMC CD3+CD161+, CD4+CD161+ and CD8+CD161+ cells in 51 healthy controls (HC), 41 patients with latent tuberculosis infection (LTBI) and 57 patients with tuberculosis (TB) were detected by flow cytometry, and their differences in different populations were compared. The expression levels and differences of PBMC and PFMC CD3+CD161+, CD4+CD161+ and CD8+CD161+ cells in 21 patients with tuberculous pleurisy (TBP) were compared. Results The expression of CD161 in PBMC and PFMC was detected by flow cytometry, and its expression in CD3+, CD4+ and CD8+ T cells was analyzed respectively. The expression of CD3+ CD161+, CD4+ CD161+ and CD8+ CD161+ cells in TB was lower than that in HC and LTBI. The expression of CD3+ CD161+ and CD4+ CD161+ cells in PFMC of TBP was higher than that in PBMC (P < 0.01). The expression of CD8+ CD161+ cells in PFMC was higher than that in PBMC (P < 0.01). Conclusion CD161 is expressed in PBMC and PFMC, and CD161 can differentiate tuberculosis patients, latent infections and healthy people. In addition, CD3 + CD161+ and CD4+ CD161+ cells are locally increased in tuberculosis infection. The results suggest that CD161 can be used as a molecular marker for early diagnosis of tuberculosis.
